Surgical indications

In the case of a complete distal rupture, the tendon cannot heal optimally and effectively on its own.

Surgical indications apply only to young and/or very active patients.

The pectoralis major tendon is not essential for basic shoulder function.

There are complete distal tendon injuries, which are surgical, and more proximal, partial, intramuscular injuries, which are not surgical.

Operative timing

It is preferable to operate as early as possible (within 6 weeks), as surgical repair is then easier.

Pectoralis major injuries can sometimes be repaired outside the emergency window, several months or even several years after the initial rupture.

Pre-operative steps

When you come in, you will be seen for clinical assessment and ultrasound by Dr Renaud Guiu.

Surgical procedure

Surgery is performed as day surgery — there is no overnight hospital stay, and you return home the same evening.

The surgical incision is 6 to 8 cm long, along the deltopectoral groove.

Post-operative course and rehabilitation

Immobilisation with the elbow against the body for 4 to 6 weeks, depending on the degree of retraction.

At 12 weeks, the repair is considered strong enough to begin progressive muscle strengthening.

Return to sport, depending on the discipline, between 4 and 6 months.

Complications

There are no complications specific to this surgery. The main risks are haematoma or infection.

Résultats et conclusions

The majority of patients operated on promptly after a complete rupture are able to resume their sporting activities as before the operation.
